Advanced Directives form - The form to be completed by a person to out line their wishes. Per a mandate from the Wyoming Legislature, the Aging Division is to make available the most current approved version of the State's Advanced Directives. 

Advances Directives describe two kinds of legal documents, an individual’s instruction and a power of attorney for health care. These documents allow a person to give instructions about future medical care inc ase they are unable to participate in medical decisions due to serious illness or incapacity.

Assisted Living Facility Waiver Provider List - This is a list of all Assisted Living Facility  Home and Community Based Services Waiver Providers.  It includes where they are located (with their full address), pertinent numbers, and services each provider provides.

Assisted Living Facility (ALF) Waiver Supervisor Report -  Form used by supervisors to report quarterly information about their ALF Waiver case manager's activities.

HCBS Waiver Renewal Application - Wyoming has submitted the application for the third renewal of the Long Term Care Home and Community-Based Services Waiver (LTC/HCBS) to the Center for Medicare and Medicaid Services (CMS) for approval. The application in its entirety is available on this site for review.
