Home Health is a Medicaid program that provides services to persons in their homes under a physician’s plan of care.
Hospice is a Medicaid program that provides care related to the terminally ill delivered in a patient’s home under a physician’s order.
Vereen Bebo is the Aging Division’s contact person for these two programs.
Nursing Home is a Medicaid program for those individuals with medical needs who are unable to continue to live in the community.
State Licensed Shelter Care provides payment of nursing home services for persons who are unable to pay the cost of nursing home care, and who are not eligible for Medicaid.
PASARR is Pre-Admission Screening and Resident Review. This is the Federally required program to evaluation individuals with Mental Illness and Developmental Disabilities for appropriate placement in nursing homes.
