Survey/Certification Unit

The Survey/Certification Unit includes a Waiver Certification Specialist located in the Cheyenne office and two Survey/Certification teams.  Each team consists of two Qualified Mental Retardation Professionals (QMRPs) and a registered nurse.  The Unit is responsible for:

·        Completing annual recertification of over 930 Home and Community Based Services Waiver providers.  Please click on the link “DDD Forms” and choose Recertification Information” for more information on this process.

·        Completing the initial certification of new waiver providers.  If you are interested in becoming a waiver provider, please click on the “Area Resource Specialist Unit” link to find the contact information on the Area Resource Specialist that covers your area.  They will explain the process for becoming a provider and get you started. 

·        Managing the Division’s Complaint Process including completion of complaint investigations.  If you have a formal complaint, please contact our main number at 307-777-7115.

·        Managing the Division’s Notification of Incident process including completion of follow-up on critical incidents.  If you need to file an incident report on the web, please click on the link labeled “INCIDENT REPORTING FORM” on the left of the screen.  To get a copy of the incident reporting form, please click on the link “FORMS’, and go to “INCIDENT REPORTING FORM.”
