Register your Classes

• Register your Medication Assistance Training Class
  (Must be submitted 2 weeks prior to your scheduled class date; if you have an emergency
  and cannot register your course 2 weeks in advance - you still need to register your course.)

 

Obtain Forms & Documents  
Roster, Survey, and Final Exams are submitted 3 days after your training to:

• Medication Assistance Training Roster
• Medication Assistance Training Survey  
• Medication Assistance Trainer Agreement
  (Above is Copy - original completed and collected at your Trainer MAT course by Division RN)
