Division Site Recertification Reports of CARF Providers

This section contains the certification reports for CARF accredited organizations for the past four years. Please click on one of the years for an index of the available reports for that year.

The Division also certifies approximately 900 Non-CARF or self-employed providers. If you are interested in obtaining a certification report for a Non-CARF provider, or a provider not listed please contact 307-777-7115.

If a provider is disputing any of the recommendations in the final report, the report posted on the web will note the recommendations under dispute. However, the final report will not have deletions or changes made until the dispute is reviewed and a determination is made.
