How Do I Apply?
You can complete and submit an application online by using the Healthlink website at, https://healthlink.wyo.gov/.
If you are unable to complete this process online you can print the application from the link below and return to us by fax or mail. You can also request a paper application be mailed to you by calling our office at 1-888-996-8786.

Applications are also available at County Health Departments, Department of Family Services, schools, healthcare facilities, Head Start programs, and Indian Health Services.

Please be sure to complete all areas of the application. Applications must be signed. Incomplete applications will be delayed in processing until they are completed.
Children are enrolled for coverage lasting 12 continuous months. As long as eligibility requirements continue to be met, your child/children can be renewed each year until their 19th birthday.
Kid Care CHIP will not turn down a child due to pre-existing conditions.
