Assisted Living Facility and Long Term Care Waiver
The Assisted Living Facility Waiver offers an option for individuals who need nursing home level of care but wish to remain in the community in an assisted living facility and receive specialized services that may delay or prevent admission to a nursing home.

What do the Assisted Living Facility & Long Term Care Waiver programs cost?
A recipient of the Assisted Living Facility (ALF) Waiver is responsible for paying room and board costs under the ALF waiver. A recipient of the Long Term Care (LTC) Waiver is not responsible for paying toward the cost of services provided under the LTC Waiver. Co-payments may apply for services.
How do I apply?
Download a Medicaid Aged, Blind or Disabled Programs Application Packet (DHCF-200) or contact your local Department of Family Services office, or the Department of Health Long Term Care Unit .
What do I need to apply?
- Verification of U.S. Citizenship and identity
- Provide a Social Security Number or verification that you have applied for one
- Documentation of medical need
- Documents to verify income and resources
