Tobacco Cessation Coverage

 

Wyoming EqualityCare covers the following smoking and spit tobacco cessation products to help eligible clients quit using tobacco products:

 

·         Bupropion (Zyban® and direct generic equivalents):

o   > 84 days or 168 tablets in the last 365 days. 

·         Varenicline (Chantix®):

o   > 84 days or 168 tablets in the last 365 days. 

·         Nicotine polacrilex (gum) and lozenge:

o   > 84 days or 735 pieces for the 2mg and 4 mg strengths, as well as for the lozenges in the last 365 days. 

·         Nicotine transdermal (in the last 365 days):

o   15mg patches - > 42 days or 42 patches or,

o   10 mg patches - > 42 days or 42 patches or,

o   5 mg patches - > 14 days or 14 patches or,

o   21mg patches - > 42 days or 42 patches or,

o   14 mg - > 14 days or 14 patches or,

o   7 mg patches > 14 days or 14 patches.

·         Nicotine transdermal system

o   > 56 days or 56 patches in the last 365 days

·         Concurrent therapy with buproprion and varenicline is not allowed.

·         Concurrent therapy with varenicline and any nicotine replacement product is not allowed

 

Tobacco Cessation products are not covered for Medicare Part D eligible clients.

 

Wyoming EqualityCare Clients as well as all Wyoming Residents are encouraged to utilize the services of the Wyoming Quit Tobacco Program. This program offers tobacco cessation medications as well as support services for smokers and spit tobacco users who want to kick the habit.
