Wyoming PharmAssist is a program created by the Wyoming Department of Health, the University of Wyoming, supported by AARP, and is funded by the Wyoming State Legislature. Our goal is to provide any Wyoming citizen with an opportunity to meet with a registered pharmacist who will review all of the medications you take and your health history. The pharmacist will analyze your personal information for possible drug interactions and therapeutic duplications, and at the same time, provide you with possible ways to reduce your monthly medication bills.

Wyoming PharmAssist is not a consumer drug information hotline. If you have specific questions about the medications you presently take, please call your local pharmacist.

 

Below are links to several forms that we require you to complete and return to us so we can refer you to a registered pharmacist for a medication review:

When we receive your completed health care information, Wyoming PharmAssist will then arrange for you to meet with a program pharmacist for a one-on-one counseling session to talk about correct and safe medication use. At this time, you will be required to pay the pharmacist a $5.00 consultation fee. The Wyoming PharmAssist program will pay the pharmacist an additional $120.00 to conduct your medication review. The pharmacist will evaluate all of the information you provided on your Client Inventory Form. Please take the time to fill out the form completely. To derive the greatest benefit from your review, your pharmacist needs as much information from you as possible before meeting with you. Your personal consulting pharmacist will review all of the data you provide, and he/she will then analyze the information for possible drug interactions, therapeutic duplications, and possible low cost alternatives. The pharmacist will arrange for a face-to-face meeting with you to discuss your medication regimen.

 

After your consultation, you will receive a letter outlining your discussion with the pharmacist. All of your primary health care providers will receive a copy of this letter. It is imperative that you take a copy of this letter with you to your next doctor visit. This will enable you to have a more meaningful dialogue with your health care provider about your medication options as well as take a more active role in your health care decisions. At the same time, your provider will have a comprehensive accounting of all of your medication therapies and your economic circumstances so together you can build an effective, safe, and personalized treatment plan at a manageable cost for you.
