ASSIST Tool
The World Health Organization created a substance use evaluation survey called the ASSIST v3.0 (Alcohol, Smoking, and Substance Involvement Screening Test). This instrument was designed to help determine an individual's substance use patterns and assess his/her risk of harm. We have created a printable form based on that tool so that you may assess your own (or, assess someone else’s) risk of harm.

The Wyoming Department of Health, Mental Health and Substance Abuse Services Division has created a web-based version of the ASSIST Tool. This version will score the questionaire for you and give you the results. Click Here to take this test.
The Division also has a "paper and pen" version for you to take. Follow the instructions below to take the ASSIST on paper. *Note: if you use this version, you must add up your own score. If you would rather take the automated version, see above.
Instructions:

2. Answer the questions and tally scores for each substance using the Answer Sheet.
3. Based on your score, determine the effects on your health by substance. The Health Risks sheet lists common conditions associated with each substance.
