Adult Day Care Facility - Wyoming Licensure Information

The definition of an Adult Day Care Facility is any facility not otherwise certified by the Wyoming Department of Health, engaged in the business of providing activities of daily living support and supervision services programming based on a social model, to four (4) or more persons eighteen (18) years of age or older with physical and mental disabilities.

Adult day care is a community-based group program designed to meet the needs of adults with physical or mental disabilities through an individual activity care plan. It is a structured, comprehensive program that provides a variety of health, social, and related support services in a protective setting during any part of a day but less than 24-hour care.

A valid state license is required prior to providing care to any client/patient/resident.

Following are the requirements and procedures through which your agency may be approved to be licensed as a provider of Adult Day Care Facility services. 

Click below for a license application for State licensure.  Please fill out the application in its entirety and submit to our office, along with the required fee.  After reviewing the regulations, please contact an engineer at (307) 777-7123 to discuss the building requirements and any construction review approval process.   Once we have approved these items, a provisional license will be issued, and you will be able to start providing services to clients/ patients/residents.

An initial health survey will be conducted by the Aging Division.  If you have questions regarding the health survey, call (307) 777-7986. Once our office has been notified by the Aging Division that the initial survey has been completed and the facility is found to be in compliance with the regulations, a permanent license will be issued.

A license permits an applicant to operate an adult day care facility in Wyoming. It does not enroll or certify a provider or supplier of healthcare services to participate in federal certification programs (Medicare/Medicaid).
