How to apply for the WCCEDP

 

WCCEDP Enrollment Qualifications:

  • Age 50 or older (unless a physicaian recommends screening befpre age 50);
  • At or below 250% Federal Poverty Level;
  • Uninsured or underinsured (current insurance does not cover entire costs related to screening); 
  • Patient of a clinic or hospital participating in the WCCEDP; and
  • Resident of Wyoming for at least one year.

If you fit all of the above qualifications you will then continue with the following steps:

Step One:

Please fill out and sign an Enrollment Application, an Affidavit of Residency, an Infromed Consent, and a Release of Medical Information and submit to the WCCEDP via fax or mail.

Step Two:

If you are accepted you will recieve an approval letter, a VOUCHER, and an enrollment packet.

Step Three:

You will receive a phone call from the WCCEDP nurse to assist you in scheduling your pre-operative exam and colonoscopy with a WCCEDP enrolled provider and recieve further education and information during this phone consultation.

Step Four:

Once you recieve your voucher, you will submit your voucher to the WCCEDP approved physician at your pre-operative consultation.

Step Five:

Once you receive your colonoscopy results from your doctor, you may contact the WCCEDP with any questions or concerns that you may have.

Step Six:

You will receive a questionnaire about your screening experience and the service you received. Please be honest so we may better serve other Wyoming residents.
 

Services Not Covered by the WCCEDP include:

  • Standard or Immunochemical Fecal Occult Blood Test (FOBT)
  • Stool Mutational Analysis
  • CT and MRI Colonoscopy (Virtual Colonoscopy)
  • Screening tests requested at intervals sooner than are recommended by program guidelines
  • Medical therapy of Inflamatory Bowel Disease
  • Genetic Testing
  • Sigmoidoscopy - Flexible, Diagnostic, with or without collection of specimen(s)
  • Sigmoidoscopy with single or multiple or cold biopsy
  • Sigmoidoscopy with removal of tumor(s), polyp(s), or other lesion(s) by hot biopsy forceps or bipolar cautery
  • Sigmoidoscopy with control of bleeding
  • Adverse events or complications from colonscopy
  • Treatment for colon cancer
