Defining the Burden of Diabetes in Wyoming

The Wyoming DPCP concentrates on defining the burden of diabetes by assessing prevalence and incidence. The program collects, analyzes and distributes diabetes data from the following sources:

• Behavioral Risk Factor Surveillance System estimates prevalence and examines trends statewide and county-specific data to support planning at both levels;
• Wyoming Vital Records examines the number of diabetes-related deaths;
• Wyoming Hospital Discharge Data examines the number of hospital discharges associated with diabetes;
• End-State Renal Disease (ESRD) Data tacks new cases of ESRD and patients undergoing dialysis in Wyoming;
• Indian Health Services Diabetes Registry tracks individuals that use the Wind River Indian Health Service clinic;
• Wyoming Medicaid Claims data provides information on the number and costs of claims filed by persons with diabetes;
• Medicare Quality Improvement Organization provides Medicare claims data on persons with diabetes; and
• Diabetes Quality Care Monitoring System monitors diabetes care for individual patients and tracks care for the overall practice of health care providers and educators, for the purpose of quality improvement initiatives.
