Community Grants
Community-based projects for achieving “
best practices”
in diabetes and heart disease care and prevention
Purpose:
The purpose of the community grant program is to provide funds to help communities to
implement and sustain best practices in diabetes and heart disease care and prevention
at the community level in the state of Wyoming. Approximately $15,000 is available to
fund community grants.
Eligible applicants:
Eligible applicants will be community-based and not-for-profit organizations, including
hospitals; medical clinics; public health offices; education centers; Cooperative
Extension offices; and others.
Goals:
The Wyoming Diabetes Prevention and Control Program (WDPCP), in partnership with
the Wyoming Heart Disease and Stroke Prevention Program, seeks to fund
interventions that will lessen the burden of diabetes and heart disease on Wyoming
families by focusing on prevention and quality care. Successful grant applications will
be those designed to fill the gaps in public health services and that achieve measurable
outcomes by reducing risks associated with diabetes and heart disease. The criteria for
projects include the following:
