PATIENT REPORTING INVESTIGATION SURVEILLANCE MANAGER (PRISM)
PRISM is the communicable disease data system for the State of Wyoming, STD/TB/Viral Hepatitis program. This system allows providers and clinical staff to input their disease reports and patient information directly into the state data system, cutting down on paperwork and time, as well as providing a secure and confidential way to report required surveillance data. The system is a valuable resource for clinics, providing a means for integrated comprehensive and thorough patient and partner care based on prior episodic information.
For Public Health Clinic staff who have taken the PRISM A and B trainings, see below for PRISM user resources, PRISM Confidentiality and Usage Agreement, and Training overview.
Please contact Julie Tarbuck for initial PRISM registration, additional user information, upcoming training information, and technical assistance.

PARTNER TREATMENT PACKET INSERTS
For use in your clinic Partner Treatment Packets, we have included our Partner Treatment informational inserts below (created in collaboration with PartnerCare.org)
CLICK HERE TO VIEW THE PATIENT FLIER. CLICK HERE TO VIEW THE PARTNER FLIER
These fliers are included with each requested EPT packet sent by the WY State Health Dept STD Program. The State Program provides both an informational flier for the PARTNER and one for the PATIENT as well, detailing how best to talk to his/her PARTNER in regard to a positive result, what to do with unused medications, who needs to be treated and/or informed, what partner therapy is and how it works.
We encourage feedback on these fliers from our clinics, as well as from your patients and their partners. Feedback will be forwarded to PartnerCare in order to improve and update the information contained in these new fliers as needed.
For information about participating in the STD program's EPT and Antibiotic program, contact Canyon Hardesty.
If you are already a participant, click here to access the antibiotic/EPT order form.
