To request a hard copy version, please send an email through the TB Supplies email (tbsupplies@wyo.gov) with the header "TB Manual Request" and include your clinic name, attention to:, address, and phone number.
NEW TB MEDICATION PROGRAM REQUIREMENTS
As of August 1, 2011 all clinics requesting TB medications must have the new Risk Assessment form completed with the patient's Social Security number. If the client does not have a Social Security number, please call the Wyoming Department of Health, TB Program prior to completion of the form.
SHOULD YOUR HEALTHCARE FACILITY BE CONDUCTING SCREENING FOR TB??
All healthcare facilities should have current policies in place to address TB screening, based on the November 2010 Wyoming Department of Health recommendations (CLICK HERE).
Please read and complete the CDC Guidance and Risk Assessment (below) to determine specific facility risk and appropriate screening frequency.

SPRING ADVISORY COMMITTEE MEETING
The next meeting of the Wyoming TB Advisory Committee will take place in Sheridan, WY on May 17-18, 2012 at the Sheridan Memorial Hospital.
All interested are welcome to sit in on the call.

This new Risk Assessment form will be required for all clients receiving state provided PPD.
