In-home and Out-patient Services
VA In-home and Out-patient Services:
The Cheyenne VA Center administers a Homemaker/Home Health Aide Program. The program provides in-home services to veterans in most of Southeast Wyoming. Homemaker services include housekeeping, laundry, meal preparation, and grocery shopping. Home Health Aide services include bathing, dressing, toileting, feeding, continence, aid in ambulation, medication education and monitoring, blood draws, etc. For information about the program, contact Jan Anderson at Cheyenne VA Center at 778-7550 ext. 7652.
The Cheyenne VA Center also administers a Home-based Primary Care Program that provides skilled nursing services to veterans within a 30 mile radius of Cheyenne. Services are provided directly by VA staff. For information about the program, contact Jane Prysock at the Cheyenne VA Center at 778-7550 ext. 7740.
The Cheyenne VA Center also administers a TeleHealth program. With funding from this program a device is attached to the veteran’s phone line, and each day the veteran answers specific questions about his or her health status. The information is sent to a central database and is reviewed by a VA employee. If necessary, a phone call is made to the veteran. Advice may be offered over the phone, or the veteran may be asked to come into the VA center for examination and treatment. Currently, TeleHealth services are offered for several health problems including chronic obstructive pulmonary disease, congestive heart failure, diabetes, hypertension, and polypharmacy. Non-VA In-home and Out-patient Services:
Local home health agencies are an important source of in-home care for veterans. Most home health agencies provide skilled nursing care, and some provide additional services such as medication management, personal care, etc. Assistance with paying for services may be available from a variety of federal, state or local programs (click here for a description of major programs). Most home health agencies also accept private insurance payments. A list of licensed home health agencies in Southeast Wyoming is provided below.

In addition to the licensed home health agencies, a number of organizations provide in-home services that may not include skilled nursing care. Some of the services offered by these organizations include housekeeping, meal preparation, personal care, chore services, transportation, case management, respite care, assistive technology devices, and personal emergency response systems (PERS). It should be noted that many of these services are "non-medical". Nevertheless, they are important components of the continuum of care necessary to avoid pre-mature nursing home placement. A list of in-home service providers in Southeast Wyoming is provided below.
Government funded out-patient services are offered at a reduced cost at a number of locations. These include public health offices and several community health centers (CHCs). The largest CHC in the state is the Community Health Center of Central Wyoming located in Casper. It is funded largely by the Bureau of Primary Health Care (BPHC). Adult Day Care Services
VA Adult Day Care Services:
There are no VA sponsored adult day care services in Southeast Wyoming
Non-VA Adult Day Care Services:
Adult day care services are in rather short supply throughout Wyoming. The following table shows the locations of adult day care providers in Southeast Wyoming. Boarding Home Services
VA Boarding Home Services:
The Cheyenne VA Center provides funding for a Homeless program that offers boarding home services to veterans in Southeast Wyoming. Fifteen beds are available in Cheyenne. Veterans can stay in these facilities for up to two years while receiving counseling, job training, medical treatment, etc. For information about this program, contact Larry Melka at the Cheyenne VA Center at 778-7353.
Non-VA Boarding Home Services:

Assisted Living Services
VA Assisted Living Services:
There are no VA supported assisted living services in Southeast Wyoming.
Non-VA Assisted Living Services:
There are 19 licensed assisted living facilities in Wyoming. The table below provides the locations of those in the southeastern part of the state. Even though it is not located in Southeast Wyoming, the Veterans Home of Wyoming, located in Buffalo, is also included in the table. It is a state funded assisted living facility developed specifically for veterans. For information about this facility, contact Jack Tarter at (307) 684-5511.
Financial assistance for assisted living services is sometimes available through the Assisted Living Facility (ALF) waiver. This Medicaid waiver provides funding for approximately 146 individuals at a single point in time. Nursing Home Services
VA Nursing Home Services:
The VA offers nursing home services in only two Wyoming facilities located in Cheyenne and Sheridan.Generally, services at these locations are available only to veterans with a service connected disability. Both VA Centers also administer a small Contract Nursing Home Program. This limited program allows each Center to contract with private nursing homes to provide services to veterans if for some reason the veteran cannot be served directly by the Center. Non-VA Nursing Home Services:
Many Wyoming veterans receive long-term care services from non-VA nursing homes. Medicaid pays for the care of approximately two-thirds of all nursing home residents in the state including many veterans. Most of the remaining one-third of nursing home residents pay for their care with personal funds or private insurance.
